While he calls himself "Doctor", his utter lack of bedside manner (including a propensity for giggling during inappropriate events, like surgery) and complete absence of any of the trappings of a medical professional shed massive doubt on the claim. What isn't in doubt is his skills — more than one shadowrunner swears by his proficiency with scalpel and suture.

He claims to live on Bainbridge Island, though obviously doesn't have many house guests in the shadowcommunity. The fact that he prefers to meet patients near one of Seattle's waterways and arrives by boat lends some credence to the story.
